Independent Medical Evaluations:
All reports include an Impairment Rating , records preparation, review and a comprehensive physical medical examination. Depending on amount of records and case complexity, that will determine type of IME.
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable up to 10 pages, records preparation, and review (up to 500 Pages), and a comprehensive physical medical examination.
Fee: $1050.00
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of 1120 pages, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted.

INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of 21or greater pages, records preparation, and review (up to 2000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted.

To be considered an updated IME the appointment must occur within 12 months of the original IME, pertain to the same injury and injury date and be scheduled with the original doctor.

Shall be applied if the original Doctor is available but another Doctor is chosen for the appointment due to original Doctor availability or other discretionary reasons outside of the control of Commonwealth IME.

KY Form 107 Examination:
INCLUDES completion of KY Form 107, records preparation, and review (up to 250 pages), Physical examination, and an Impairment Rating where applicable. *Form 107 exam/report: Fee $200.00 (for additional clarification please refer to KY WC 803 KAR 25:160 1,3) *Outside Records Review for Form 107 Examination

Specialty Independent Medical Examinations:
All reports include an Impairment Rating , records preparation, review and a comprehensive physical medical examination. Depending on injury of client, and the required speciality, will determine type of IME and physician.
Standard (W/ Post-Concussion Protocol Evaluation):
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 10 pages (excluding survey and test materials), King-Devick post-concussion test administered, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted.

INCLUDES: Records preparation and processing up to 1000 pages and 2 hours of Doctor’s Record Review and/or consultation ($350/hour workers compensation and $600/hour Civil cases)

Additional associated fees:
Records preparation and processing each additional 500 pages over 1000 Fee $100.00 per 500 pages

Depositions:
Fee: $250.00 for the first ½ hour of testimony and $100.00 for each ¼ hour of testimony thereafter. *For additional clarification please refer to KY WC 803 KAR 25:160 1,3
Fee: $2000 for the first hour of testimony and $600.00 for each ½ hour of testimony thereafter.

Addendums/Rebuttals :

Important Note:
Appointment Cancellation Fee – Fee: $200.00, The Fee shall be applied for “No-Show” patients and cancellations which occur 5 business days or less prior to the scheduled appointment. This Limited notice does not allow us to properly replace the appointment
*Commonwealth IME reserves the right to cancel any appointment 3 weeks prior to the appointment if the medical records provided at that time are deemed inefficient to complete an accurate and detailed IME and/or KY Form 107.
* The appointment may be Rescheduled for a later date to allow you to retrieve all missing medical records, or you Keep appointment and have Commonwealth IME retrieve all missing medical records for a fee of $100
All Examination appointments, Record Reviews, and Deposition appointments are subject to a administrative fee of $150.
* The administrative fee may be considered for refund or transfer on a case-by-case basis for cancellations or changes to an appointment which occur 30 days or more prior to the scheduled appointment date.
