Independent Medical Evaluations:
All reports include an Impairment Rating , records preparation, review and a comprehensive physical medical examination. Depending on amount of records and case complexity, that will determine type of IME.
Workers Compensation Patients Only
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable up to 10 pages, records preparation, and review (up to 500 Pages), and a comprehensive physical medical examination.
Fee: $1300.00
Workers Compensation Patients Only
To be considered an updated IME the patient must have been seen by a Commonwealth IME Doctor, Have a complete IME report and the IME report must pertain to the same injury and injury date. The Updated IME may be scheduled with any available doctor.

Specialty Independent Medical Examinations:
Standard:
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 15 pages, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted.
Fee $1700.00
*Additional Misc. Doctor hours for Civil cases will be billed at the rate of $600 per hour at the Doctor’s discretion.
Standard:
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 15 pages with a comprehensive physical medical examination, Basic Psychiatric testing administered, records preparation, and review (up to 250 of prepared Pages), and Doctor/client phone consultation if warranted.

Standard (W/ Post-Concussion Protocol Evaluation):
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 10 pages (excluding survey and test materials), Basic cognative testing may be administered, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted.

INCLUDES: Records preparation and processing up to 1000 pages and 2 hours of Doctor’s Record Review and/or consultation ($400/hour workers compensation and $600/hour Civil cases)

Addendums/Rebuttals :
INCLUDES: Records processing and review up to 250 pages and a detailed letter from the Doctor. Fee: $400.00

Important Note:
Appointment Cancellation Fee – Fee: $200.00, The Fee shall be applied for “No-Show” patients and cancellations which occur 5 business days or less prior to the scheduled appointment. This Limited notice does not allow us to properly replace the appointment
*Commonwealth IME reserves the right to cancel any appointment 3 weeks prior to the appointment if the medical records provided at that time are deemed inefficient to complete an accurate and detailed IME and/or KY Form 107.
* The appointment may be Rescheduled for a later date to allow you to retrieve all missing medical records, or you Keep appointment and have Commonwealth IME retrieve all missing medical records for a fee of $200
All Examination appointments, Record Reviews, and Deposition appointments are subject to a administrative fee of $150.
* The administrative fee may be considered for refund or transfer on a case-by-case basis for cancellations or changes to an appointment which occur 30 days or more prior to the scheduled appointment date.
